Brainworks’ approach to case management is collaborative and consultative. We manage every aspect of our client’s rehabilitation program. Our focus is building relationships with stakeholders and managing strategic plans that will result in a positive improvement in our client’s functioning and well-being.

Brainworks’ case management program is comprehensive. Our case managers assess, implement, monitor and review the rehabilitation options and services required to meet our client’s individual needs. Our program is based on our philosophy of client-centred, cost-effective and solution-focused service delivery and ensures a timely and sustainable improvement in our client’s well-being. Fundamental to our case management framework is the appointment of a single, appropriately qualified professional who is responsible and accountable for managing our client’s rehabilitation file. Our case managers have extensive experience and knowledge regarding cognitive, behavioural, psychosocial and physical challenges.

Comprehensive Liaison

Brainworks’ case managers act as a liaison between the client, the medical and rehabilitation professionals, lawyers, insurers and employers to identify what services and resources are necessary to promote our client’s highest level of well-being. We meet with people in person, or by telephone, and create standardized and reflective documentation to enhance communication among all parties. We help our clients and their families navigate and understand the health care system, providing resources and support that will be needed to return to a productive lifestyle.

At Brainworks, we educate and encourage our clients to better understand their injury or illness while highlighting their abilities and functioning. We set clear collaborative goals that address physical and cognitive impairments while empowering our client’s active involvement in their own treatment and recovery.

Through our approach and commitment, we help our clients increase their functioning and independence across
cognitive, behavioural, psychosocial and physical domains.
